Self-enrollment Access my account

Our patient portal, My Health Portal, is an online tool that goes a step beyond simply displaying information by helping you manage your healthcare easily and securely. My Health Portal provides secure online access to portions of your medical record such as:

  • Lab Results
  • Radiology Reports
  • Visit History
  • Discharge Instructions
  • Health Summary
  • Allergies and Conditions
  • Immunizations
  • Medications with Instructions

 We adhere to Best Practices and industry standards to safeguard your information. You and authorized family members are the only ones who can access your Portal. Also, a timeout feature protects your information if you leave the Portal page open.

Access My Health Portal

If you are not enrolled in the portal but you have provided your email address to us during your admissions/registration process, please follow the instructions for Self-Enrollment below. You will need to have your Medical Record number in order to enroll.

If you have not provided an email address to us or need assistance with the enrollment process, please contact our Medical Records Department at (323) 725-5020, Monday through Friday, from 8:30 am to 5:00 pm.

You can Manage Your Family's Healthcare Needs Too

For loved ones in your care, you may set up a Proxy Account. Please complete the "Authorization for Proxy Access to My Health Portal" Form and bring it to the Medical Records Department. Proxy must be 18 years of age or older, and an identification document is required.
